BEFORE THE CLASS

• High School Diploma or GED (optional)
• ID/Passport
• SSC
• Livescan/Fingerprint
• AHA CPR/BLS- Basic Life Support
• Physical Exam (current)
• TB test (current)
• X-ray (copy required if tested positive)
• Vaccination Shots (must be fully vaccinated)
• Quality Care Health Foundation will also provide a Certificate of completion for the fee of $30.00 with the RNP Pin total of $30.00 money order paid to QCHF. No refund once this is sent to QCHF.
